How Can Bacterial Infection Under The Thumb Be Treated?

hello- I have a bacteria under my thumb that won t go away. It seems to clear up and then comes back. I ve been to a dermatologist three times to no avail. Finally he gave me eye drops called Gentamicin - it clears it and then comes back / this is going on at least 2 years any suggestions? Also using Hibiclens twice a day. I clean house for a living and I m thinking this is related - try to wear gloves as much as possible. I also already took internal antibiotics / maybe I need them again.
