Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

It looks like some type of allergic reaction. It could be due to some insect bite or food allergy. You complain of swelling & if it is itchy & painful it can be contact dermatitis. It seems its spreading. He needs Antihistamines, for rashes/ redness apply locally creams that contain corticosteroids. Don't let him scratch, us a high quality moisturizer or calamine lotion. If he gets better then fine, but if it spreads & the child gets febrile you need to visit ER as he will be needing antibiotics due to infection in-filtering into the lymph nodes.

Hope I have answered your query. Let me know if I can assist you further.
