How Can Chronic Constipation In A Child Be Treated?

My 5 yr old has been constipated for 18 mths. He has been cleared out on laxitives with no luck, it returns within 24hrs. He has tried lactulose but does not work or gives him the runs within an hour, and only on 7mls. His bm are often accidents that are sticky and smell but every now and then he will have a totally normall bm. We can even get 1-4 wks of totally normal. 4 mths ago he had twisted bowels which was 4 days in hospital, no surgery, and we believe there have been other times he has had this. He is small in size, always hungry and tired. Would love some ideas on what this may be as we have so far not had any luck with drs we are seeing, they seem totally baffled. He is also on a very low lactose diet.

Given the chronic nature of constipation in your son, the best treatment may not be limited to giving laxatives.The cause will have to be investigated and managed.There are many possible causes of chronic constipation.Some of these causes may include:
-Blockages in the colon or rectum like Bowel obstruction, Colon cancer, Narrowing of the colon (bowel stricture), Other abdominal cancer that presses on the colon, Rectal cancerrectocele
-Problems with the nerves around the colon and rectum like Autonomic neuropathy, Spinal cord injury
-Difficulty with the muscles involved in elimination like anismus, dyssynergia, Weakened pelvic muscles
-Conditions that affect hormones in the body like Diabetes, hyperparathyroidism, hypothyroidism

I will suggest you see a gastrologist for examination.Tests that may help identify the cause of his problem may include: sigmoidoscopy, colonoscopy, anorectal manometry, colonic transit study, abdominal CT scan and other blood tests like hormonal work-ups, fasting blood sugar...

Treatment for chronic constipation usually begins with diet and lifestyle changes meant to increase the speed at which stool moves through your intestines.I will advise the following:
*Increase fiber intake
*Exercise most days of the week
*Osmotic laxatives like oral magnesium hydroxide, lactulose ...
*Lubricants such as mineral oil enable stool to move through colon more easily.
*Stool softeners like docusate sodium
*Enough fluids
*Fruits like soft pawpaw and oranges but avoid fruits like bananas that may further constipate.
