How Can Chronic Paronychia On The Thumbs Be Treated?

I have a chronic paronychia condition in both of my thumbs, and to a lesser degree in my fingers. I recently let a metal nail file slip and it travelled the length of my left middle finger between the nail and the adjacent fold of skin. The area is now red, swollen, warm to the touch and painful to touch and sometimes painful regardless of touch. I plan to see my dermatologist when Irma passes through, but in the meantime, do you have suggestions for treatment? Anti fungal cream? Antibacterial cream? Oxistat? Use warm compress or soak in hot water? I guess I mainly want to know if you think the symptoms would warrant a prescription sooner than later? I continue to be confused as to whether this is a fungus or bacteria issue. Thank you. Tom Mozley

* The presentation of narrated symptoms suggest that there is acute inflammatory response due to current infection.
* Primary guidelines till you consult your doctor

- Keep the area clean with antiseptic liquid.
- Local application of anti bacterial cream as Polymycin or else.
- Tablet Ibuprofen 3 times a day after meals with antacids if no history of drug sensitivity.
