How Can Continuous Bleeding Mid Cycle Be Treated?

i have not had a single full week free from periods. i have always been irregular since i started wen i was 10. I have tried many different types of the pill, tranexamic acid, been to the gynocologist, to the doctors, to hospitals to everywhere been back and forth and still i am always bleeding. I am 28 and have no children. I was trying (for 4 years)but obviously with me never not bleeding how can we? I just want it to stop. I am worried. I feel sick with worry. Its getting me so down. I cant take it anymore - i want them to go away. 18 years is enough.
