Recommendation for 29 yr old patient with mild circumfential disc bulging in the L4-L5 and L5 S1 level, with mild facet hypertrophy at both level experiencing all sides of disc pressing against sciatica nerve. Patient has undergone extensive conservative treatments which have all failed.
