How Can Enlarged Adenoids In A Toddler Be Treated?

Hello, My daughter is 3+yrs. She was diagnosed with mildly enlarged adenoids at the age of 1 yr and 10 months. She has running nose for most part of the year and the doctor suggested Metatop spray. We spray once a day for about 3 weeks or so and then gradually reduce over next 2 weeks. Then there is a gap of 3-4 weeks and we have to spray again. Sometimes, there is purulent mucous and she is given a short course of amoxicillin. Worried about this constant dependence on Metatop and antibiotic.Pls advise! Regards, Jyotsna

Hello.

I can understand your concern.

Adenoids are a lymph gland which act as a policeman near the nose, filtering and reacting when necessary. Sometimes, they react more than necessary causing unwanted symptoms.

The current line of treatment is appropriate. You can also add a decongestant or antihistamine. Cetrizine is a good and safe drug for use in children. Whenever there is runny nose you can give 5mg of Cetrizine syrup once a day for a few days and then stop. Antibiotic will be needed whenever there is bacterial infection of the mucus - it cannot be avoided when there is overwhelming infection.

In addition to the above, you can give her steam inhalation twice a day. Steam induces local immunoglobulin secretion and helps in controlling the inflammation which in turn the adenoids to decrease in size. Propped up position at night also helps as the airway is open.

However, if the problem still persists, and she has trouble breathing properly due to enlarged adenoids, especially at night time, she may have to be surgically treated i.e. adenoids have to be removed.

Hoping she recovers with medications only and does not need surgery.
