the blood pressure is 170 s/110. The patient has no headache but wakes up at 3 am feeling funny and the blood pressure is 180+/110. Only been on diovan for 1 week 160mg po qd. Should he take a second dose tonight before bed if the pressures are this high again?

Hello,

The antihypertensive effect of Valsartan (Diovan) is substantially present within 2 weeks and maximal reduction is generally attained after 4 weeks.

If the additional antihypertensive effect is required over the starting dose range, the dose may be increased or a diuretic may be added. Addition of a diuretic has a greater effect than dose increases beyond 80 mg.

Due to its long time to onset for action, it won’t be effective for stat control of hypertension. The drugs that are useful for immediate control of blood pressure are labetalol, nifedipine, captopril and clonidine.

You can talk to the prescriber for use of these drugs for state control of blood pressure.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Maninder, General & Family Physician
