How Can Poor Weight Gain In A Child Be Managed?

Hi! My son was breastfed his first few weeks of life, but after his check up we discovered that he was not gaining any weight, not even an ounce in a whole week so they recommended that we start supplementing with alimentum formula. We did this while still breast feeding for a week, but I had to start back to school at 4 weeks postpartum in order for me to be able to graduate in May so we have just been doing straight formula because I have not been able to pump during my clinical rotations and it just seemed like the best fit plan at this point. We continued on the alimentum but he was pretty fussy and seemed to never be full and satisfied. He also started spitting up more frequently and would cough and be stuffy which lead me to believe he has reflux. We went back to the doctor on Thursday and I told her about it so we decided to switch his formula to enfamil AR to see if there was any improvement. The first day he was very fussy but the second day he was acting much fuller, happier, & content but still a little fussy. But there has been little to no spitting up, but he has developed a rash on his face, stomach, chest and back. He also has been having pretty runny bowel movements after each feeding and now has developed a diaper rash. I read that a red ring rash around your infant s anus, often accompanied by a diaper rash could be a sign of a milk allergy as well as the rash on his body. I’m just wondering if I should continue on the new formula for a week like the doctor had said to do or if I need to stop using it immediately! Also, could he be having an allergic reaction to the alimentum too and maybe he needs neocate or elecare.

* As per my clinical experience, the condition is due to underlying lactose intolerance with or without some enzyme deficiency or alterations.

I recommend to stop all commercial formulas at present and consult his pediatrician for further work up and guidelines.
