Since you are a known patient of varicocele, get detailed investigations:
Color doppler for knowing the grade of varicocele
Semen analysis to see the defect as you already have 4 years child mean the problem of infertility is recent.
Clinical examination by a General Surgeon or Urologist.
All these things together will lead to a proper plan for surgery.
Surgery done in a proper ways gives results.


Additional points:
No Ayurvedic treatment help, so do not complicate the matter, go for surgery.
Avoid hot and humid places.
Loose clothes.
Sitz bath in cold water,
All help a lot.
