Hello. Thanks for writing to us at healthcaremagic

For severe grade acne I usually start my patients on Oral antibiotics e.g either Azithromycin Or Doxycycline Or Minocycline. I continue them for 4-12 weeks depending on the severity of acne and response to treatment.
Besides Oral antibiotics, Oral Retinoids are also an option for moderately severe to severe grade acne. They are the most effective antiacne medications.
Oral retinoids are first choice in long standing acne Or acne not responding to traditional antiacne drugs Or acne that relapse as soon as treatment is discontinued Or acne associated with scaring.
Topical treatment is an adjuvant to oral treatment; I usually ask my patients to use either Benzoyl peroxide or clindamycin during day time for inflammatory acne.
Topical retinoids are mainly for night time application. Topical retinoids are first line treatment in all grades of acne; they are comedolytic and therefore also prevent new acne formation. They should be applied to the whole affected area once daily at night.
You may visit a dermatologist in your region. Your dermatologist will assess the severity of your acne and advice accordingly.
Chemical peeling is an option too; salicylic acid peel is quite good for acne. You can discuss in more details about it with your dermatologist.
