How Can Severe Bipolar Depression After A Manic Episode Be Treated?

I am very upset cause have a new psychiatrist , have been one for one year and she is suggesting ECT for bipolar depression. I mainly have depression, may have had only one manic, not too manic episode so feel misdiagnosed too. Am very upset with no answers. Had a lot of bad things happen in the past and feel cognitive behavior therapy would be good for me. YYYY@YYYY

Hello,

As per your clinical history is concerned, please follow like this:

1) Do a psychological evaluation by your Doctor first [psychiatrist]and he will check your metal scale and do CBC with differential and check your TSH level.

2) To confirm a case of depression you must have five symptoms like this over past 15 days’ time:
a)     Depressed mood, such as feeling sad, empty or tearful.
b)     Significantly diminished interest or feeling no pleasure in all or almost all activities.
c)     Significant weight loss when not dieting, weight gain, or decrease or increase in appetite.
d)     Insomnia or increased desire to sleep.
e)     Either restlessness or slowed behavior that can be observed by others as fatigue or loss of energy.
f)     Feelings of worthlessness, or excessive or inappropriate guilt.
g)     Trouble making decisions or trouble thinking or concentrating.
h)     Recurrent thoughts of death or suicide, or a suicide attempt.

3) Do take prescribed medications as per your Doctor's advice and do psychotherapy by your clinical psychologist only.

Do follow lifestyle modifications like this:

1) Take steps to control stress.
2) Reach out to family and friends, especially in times of crisis, to help you.
3) Do treatment at the earliest sign of a problem to help prevent depression from worsening.
4) Do consider getting long-term maintenance treatment to help prevent a relapse of symptoms.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Uday Nath Sahoo
