How Can Severe Pain Be Treated?

I am a CNA and have a para patient that takes Morphene (100mgs) every 8 hours and Oxycodene 5mg every 4 hours. Not eliminating pain very well and wondered if Ultram 325/37,5 could be also used and what dosage? He has crushed lower verts from bullet wound and in extreme pain most of the time.

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

We will try to help you in best way possible. First of all it's sad to know what you had been going through lately. Based on the history you have shared, you have already been on multiple potent analgesics and if still pain persists option of nerve blocks may be needed and pain medicine specialist should be consulted in that regard. Ultram can be used twice daily after consulting your primary care physician. Meanwhile analgesics like acetaminophen can be used along these potential ones for better results. Regards
