Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

It is difficult to say what it is, until examination is done.
He may have injured his glenoid labrum, pulled muscle or tendon rupture etc. Until examination is done it is difficult to say what it is.

Please consult your physician/orthopedician they will do MRI of shoulder joint and also evaluate the cardiac cause of left arm pain and treat you accordingly.

I hope I have answered your query. Let me know if you have any further questions.
Regards,
Dr. Penchila Prasad Kandikattu
