How Can Severe Pain In The Muscles Be Treated?

I had tri-level lumbar fusion two years ago. Surgery was good for the spine issues, but seriously damaged multifidus muscles and related muscle coordination. Doing lots of PT, it helps, but those multifidi are almost always seizing spontaneously now. I have done Ibuprofen all the while. Tramadol recommended, have taken it, but have unpleasant mental side effects. Which of the stronger Ibuprofen meds might you recommend, if any? Problems with taking them? would they help more with the off and on muscle pain, level 4-7?
