How Long After Intravenous Drug Use Does Symptoms Of Endocarditis Show?

Hi I am agoraphobic and have constant panic attacks, almost two yrs ago I moved to a new town where no doc takes me seriously and as a result I no oonger recieve medication. The last doc I saw told me since im alive there is no reason for me to have depresion and gave me propranalol ive just read that I shouldnt take it if I have depression and olso because I have raynards phenomonon. I had endocarditice in 2012 and stopped using drugs but the panic attacks got so overwhelming a coupke of days ago that I used yesterday my hands and feet became freezing tingling and sore spots on my feet, I have tachycardia but im having massive panic attacks at the idea of going to the hospital with this. How long after intravenous drug use would I see symptoms of endocarditis and I dont have a fever. Im unable to controle my panic attacks now as im terrified my one stupid moment may have caused this again and the thought of having to explain this at a hospital has me hqving suck severe panic that im dizzy and have to stay laying down or I think ill pass out.

Thank you for your question! I understand how do you feel, but at the first moment I advice you to calm down for a while because your clinical situation doesn't seem problematic at the moment you don't look to have clinically evident cardiac or pulmonary complication, no abnormally high temperature at all; it is true that using iv drugs is one of the minor criteria (Duke criteria) for the diagnosis of right sided endocarditis, but to rule out this phenomena at the face you have experience such clinical problem before the only thing to do for your routine check up, which would relief your anxiety it to perform an cardiac ultrasound (ECHO). By ECHO the right atrio-ventricular valve (tricuspid valve) can be studied for the presence of possible vegetation, to compare with the previous possible ones, to control for any abnormally new tricuspid insufficiency, etc.
Regarding the time needed for a pathogen agent to exert a clinically evidence of endocarditis it depend on tne type of pathogen (staphylococcus, fungi, gram negative organisms, etc) and and on the presence of previous valve envolvement grade..
so to cut it short and clear for you I recommend you to perform an ECHO.
