How Much Potassium Can Be Increased By 15mg Lisinopril?
Hi. I am a pharmacist and when I recently had labs done my my potassium was 3.1! When my Physician put me on K-dur 20MEQ QD for a week and re-drew blood my Potassium was spot on 4.5 and I never felt better. She then had me come in a week later, off RX K+ and re-drew my blood and found I had a barely therapeutic 3.7 K+ level. I expected her to put me on K-Dur at some level but since I am on all of 15 MG lisinopril she keeps saying that it will raise my potassium too high (keep in mind max daily dose of lisinopril is 80MG/24 hrs.) However, since I am not going to change my diet to any substantial degree she says that I should take an OTC Pot. supplement (= low bid, non-standardized and unpredictable release) without following me up with labs. After taking OTC K+ for 2 weeks, I started having chest pains for a whole day with few intervals between them. When I discontinued the OTC Potasssium, the chest pains were gone the next day. I know that Hyperkalemia can cause chest pains just as hypokalemia can. Precisely how much might my potassium be raised by a mere 15MG lisinopril per 24 hours? My doctor is young and seems very much in fear of my being hyperkalemic but I ve never been so, just the opposite. The scenario seems absurd. What is a ballpark figure for how many mm Hg BP increase might correspond with 15MG lisinopril QD? Thanks.
I'm not sure about your question, but 15 mg of lisinopril (the usual maximum dose would be 40 mg) might increase potassium by 0.5 to 1.0, depending on kidney function and diet. 3.7 would be considered a normal level, and your doctor is right that hyperkalemia is much more concerning than hypokalemia.
