How Should Chronic Shoulder And Neck Pain Be Treated?

Hi I have chronic paain in my left shoulder neck area. This is usually managable and on occasion appears absent. However, sometimes the pain increases in intensity which may last days/weeks. I am in so much pain when this occurs usual techniques such as distraction, pain killers offer no relief. My body will also strrt to tremble from the pain. What is going on?
