How To Change The Dosage Of Hydrocodone?

I am a 2 time knee surgery patient and I have been having problems since my last surgery in July 2010 my orthopaedic found Chrondromalacia, Severe OA and Grade 2 and 3 Chrondral changes and I am on Hydrocodone 5 mg but its not really helping and its wearing off faster and I want to talk to my physician about changing the dosage but I don't know how to bring it up and I am only 23 yrs old. please help!
