How To Diagnose If A Head Injury Could Lead To Long Term Cognitive Impairment?

can a head injury (concussion) cause long term cognitive issues? Back of my head hit cement very hard from a fall. I did not loose consciousness but my head had a buzzing sensation and my vision went dark. My vision changed drastically and i have had severe memory problems. This was a year and a half ago. i had a previous concussion about 9 years ago that left me with reading retention and facial recognition issues. Are these now permanent issues, is there anyway to get better?
