1. As you have tried everything and still not getting desired results check with your metabolism hormone i.e. Thyroid which can result in increased metabolism [especially hyperthyroidism] resulting in good appetite but the person not gaining weight.

PS. ask for TFT [thyroid function test]

2. principle is to have a well-balanced diet with more proteins and more calories and good exercise.
3. try to increase the quantity of food by eating half or one roti extra than the usual.
4. for proteins take more of mil with protein powder, more of cereals and legumes, Daal rice, egg, meat.
5. 200Cal/kg of expected body weight, small feed every 1/2 hour.
6. undiluted milk thickened with 1 tsp of skimmed milk powder, take a banana, potato, sweet potato, pulses.
7. Consult with your Local Physician/Dietitian regarding Appetizers, general tonics, and multivitamins.

8. it can be due to inheritance, faulty food habits, increased physical activity with decreased energy(food) input, or if suffering from any problem related to digestion?
9. sometimes low hemoglobin or worm in the stomach is also the cause of not gaining weight, check your Hb% level and stool test.
10. Mango shake is an ideal drink for you as one is free from protein (mango) and one is free from sugar (milk).
11. Don't let yourself stressed out, take a diet rich in calcium (milk, curd), Vitamin D (milk, exposure to the sun), magnesium (green leafy vegetables).
12. do compound exercises, increase protein and complex carbohydrates in your diet, sleep a lot (which will conserve energy needed for awakening).
13. make sure you are gaining weight on muscles and not FAT, so apart from eating also follow a good weight lifting program
