How To Treat Bruise And Swelling Of Toe?

I had bunion surgery (no pins) on 9/16 about 3 weeks after I fell down the stairs and hyperextended my great toe and the area was very bruised and swelled after. Since the fall I have been into see my dr and he said to wear my ortho shoe over the weekend and still I have no relief from the pain when I walk I walk on the outside of the foot. my dr did not xray and moved my toe up and down assured me i did not fracture it. I don't understand why it is about 3 weeks since my fall and it hasn't gotten anybetter any suggestions? I work long hours and am on my feet constantly due to go back next weeek.
