How To Treat Numbness And Pain In Forearms And Hands?

my forearms and hand get very numb and painful also they feel swollen but aren't when this happens its coupled with joint pain in my hands. this occurs mostly while I sleep I wake up multiple times a night in pain and have to stretch them out and put them on something cold to alleviate the symptoms although sometimes when im using my hands during the day doing different activities. I am a 28 yr old man in generally good health what might this be?
