I Have A Patient Who Is On Glimepiride 4 Mg.

I have a patient who is on Glimepiride 4 mg. with breakfast and evening meal, metformin 500 mg with breakfast and lunch, and 1000 mg with dinner, and januvia 100 mg. daily. She has low BGs between midnight and 2 a.m. and wakes up with high BGs 171 - 239 mg/dL. She eats very regular meals. Do you have any thoughts on how to prevent the low BGs during the night, and highs in a.m.? I am a nurse diabetes educator, and the patient's doctor is out of the office today.
