I'm Not Sure If This Is In Your Area &

I'm not sure if this is in your area & this seems confusing, so I will apologize in the beganing. :-) I am seeing a Psychiatrist since 2011. He retired in May of 2016 & that was my last appointment with him. Another Dr. has came into the practice &I am now seeing her. In Jan 2012 I took a Clinicom Test for ADHD, it was positive & was put on adderall along with xanax, wellbutrin xl (take this in AM) & wellbutrin sr (take this in PM). In Nov 2012 I had to take a paper ADHD checklist & I am still taking the same meds. My concern is the Dr that orginally diagnosed me did not put this in my Medical File, it only has that I am major depressed. Verbally I was told this, I have filed for disability thinking that I was diagnosed with ADHD, major depression, migraines, carpal tunnel, osteoarthritis & Fibromyalgia. Which brings up another issue.... but 1st, ADHD, how do I go about getting this new Dr to add it to my file. When I was her, ( the 18th of this mth is 3rd visit) she said she didn't see anything & could be in my file she would have to research it. I called leaving messages to remind her b-4 my 2nd visit & on my 2nd visit she had not looked in my file & told me my time was up we would look into it the next visit. I called med records today & asked if a message could be left for her to check out the file b-4 next visit. I saw my therapist yesterday & told her too but there isn't much she can do. Do you have any suggestions for me to try for my Psychiatrist.
The other issue with Fibromyalgia.... my family Dr blood test showed RA factor of 39 on Sept 2015, my Rheumatologist took blood Aug 2016 & now my RA factor is 77 & he said I do not have Rheumatoid Arthritis but I have Fibromyalgia & Osteo-Arthritis, possibly could progress to Rheumatoid later. This is a really high number for a RA factor & he repeated himself. I'm so confused. Can you maybe make some since out of this please? I am 57 yrs old, termaninated from my job after 14 yrs & 1 day, (I believe because I asked alot of questions and they complained of me being slow butl I was accurate!! seldom was there any errors, I was a loan officer at a Credit Union) located between Wetumpka, AL & Alexander City, AL. I'm also OCD but never diagnosed. ....
