Irregular Periods, Painful Cramps When Cough Or Sneeze. Pregnancy Test Shows Positive. Noticed Dark Spots. Is It Normal?

I have a doctors appointment in two days, but have some concerns. I've had multiple positive home pregnancy tests, however due to irregular periods I can't really map out how far along it would be.
I've had some pretty painful cramping when coughing or sneezing, not every time but often. Today I noticed some dark spotting in the ladies' room. How "normal" or alarming is this? Should I see a physician sooner?

In case of irregular periods, following options help in detecting the gestational age.
-Ultrasound
-Blood test for pregnancy
-Date of fruitful coitus or the date when urine test for pregnancy came positive.

Bleeding in early pregnancy can be seen due to many causes like, implantation bleeding, due to miscarriage etc.

So, once consult gynecologist and go for ultrasound.
Ultrasound helps in detecting the gestational age and to rule out possibilities like miscarriage etc.
Then with her advice, you can go for progesterone supplementation.
