Is Being Bitten By A Dog Post Anti Rabies Vaccine A Cause For Concern?

Hi Sir,

My nephew who is 4 years old, was bitten by our puppy two weeks ago and he had his anti rabies vaccine last monday this week for the first two shots, then on wednesday another two shots, and then on monday for the last two shots. but my question is, he was bitten again this morning by our puppy, is he still safe because he already taken some of the vaccines? or do we still have to add another shots of vaccine? Please enlighten me. Thank you.

General & Family Physician 's  Response
Hi,
Thanks for posting in HCM.
I understand your concern.
As per the WHO guidelines, the following is the guide for post-exposure prophylaxis in case of contact with animals such as dog.
Post-exposure treatment consists of local treatment of the wound followed by vaccine therapy. It should be initiated immediately with contacts of categories II and III.
Types of Categories are:
1. Category I – touching or feeding animals, licks on the skin.
2. Category II - nibbling of uncovered skin, minor scratches or abrasions without bleeding, licks on broken skin.
3. Category III – single or multiple transdermal bites or scratches, contamination of mucous membrane with saliva from licks; exposure to bites or scratches.
From your description it appears that the injury appears to be falling in Category III.
However, treatment may be discontinued if the animal involved remains healthy throughout an observation period of 10 days; or if the animal is killed humanely and found to be negative for rabies by laboratory examination.
Hope the information provided would be helpful.
All the best.
