Hi, thanks for posting your concern in the HCM. A fluctuating BP level with sudden rise of systolic bp to 188 mm of hg is a matter of concern. You also have an increased pulse pressure. A high pulse pressure may occur in advanced atherosclerosis, hyperdynamic state, eg., anemia, thyrotoxicosis, vulvular regurgitation. On the other hand a fluctuating bp may result from anxiety, poor adherence to bp medications, exercise, sleep etc. However, in the absence of a physiological reason, it is definitely a matter of concern. It may be caused due to DM, pheochromocytoma, parkinsonism, autonomic nervous system diseases etc. Also it increases the risk of stroke for the duration for which your bp remains high. Thetefore, please consult your local doctor at the earliest. For any further information please write back to us. Regards. Dr. Kaushik
