Is Having Spina Bifida Occutla With Chron S Disease Serious?

My boyfriend is 21 years old and has Spina Bifida Occulta as well as Chron s Disease. He refuses to get regular check ups and he drinks and smokes as well. Would having these 2 conditions effect him in the future? Any information will help me cope with the situation. Thank you
