Is Heart Rate Above 140 A Cause For Concern?

My husband is 74 and was recently diagnosed with atrial flutter and occasional atrial fibrillation. He is on blood pressure medications and beta blockers. Sometimes his heart rate is above 140. He has suddenly begun waking at night due to inability to breathe. What should we do?

Elevated heart rate with chest pain/pressure/breathing is an immediate emergency and a reason for even an ambulance. Beta blockers are supposed to prevent that from happening.
