Is Heavy Bleeding During First Menstruation Normal?

my sister is 11 years old and has started her period yesterday, today when she woke up she is bleeding heavy and is in a lot of pain. Just now she went to the toilet alot of blood came out along with a huge blood clot came out... is there anything to worry about.
