Is Hyperechoic Focus At The Upper Pole In An Ultrasound Of The Abdomen Normal?

Hi doctor, I got an ultrasound in abdomen and it shows Kidneys: No pelvicalyceal dilatation. The right kidney measures 9.4 cm in length. It contains a hyperechoic focus at the upper pole measuring 4 mm in diameter. The left kidney measures 9.0 Result impression cm in length. What does it mean??? Thank you.

Hi,

Your kidney lenghts are at the lower limit of normal size. It may be just normal for your age if you are older than 55-60. But I still suggest you to have a blood test for your kidney function eventhough if you are older than 55-60.

Hypererchoic focus at the upper pole generally have two common meanings: (1) If hyperechoic focus is in renal paranchyma (not in the collector system) it is most likely angiomyolipoma (90-95%). But renal tumors can be seen as a hyperechoic focus when they are very small but it is a very low probability.

So I suggest a control ultrasound after 3-6 months, or if there is high sonographic suspect about the lesion (irregular border, heterogeneous echopattern) an MRI can be helpful to distinguish.

(2) If hyperechoic focus is in collector system or renal pelvis, it may be a small kidney stone or calcium milk. Just reporting hyperechoic focus on upper pole is not very specific. Is it in paranchyma or in collector system?

Does it have regular or irregular border? Is it homogeneous or heterogeneous? I suggest a control ultrasound and detailed information about the lesion. Then we can consider following the lesion after 3-6 months or getting an MRI for the lesion. Don't forget to have a blood test for kidney functions.
