Hi,
Welcome to health care magic,

If the hernia repair surgery has been done 7 years ago then I do not think there is any chance of its involvement in this testicular swelling or scar pain and burning.

It could be some other infection of the testis and the burning repaired area of hernia surgery.

I think you should go for blood works,ESR and ultrasound of local part to rule out infection and fluid collection.

Meanwhile, you can take painkillers with serratiopeptidase orally or topically for symptommatic relief and you may also require oral antibiotics.

So,it would be better you should go to surgeon who will start treatment after examination and relevant tests.

Regards,
