Hi,
It is really painful to know the situation of your sister.

As you have mentioned your sister has Lupus and there is renal problem which require dialysis ie end stage renal disease.

First we should know what is lupus-
It is not just a blood disorder.
It is a multi system disorder described as autoimmune / connective tissue disorder.
There is involvement of multiple organs in the body (can involve any organ) but most commonly involved organs are kidneys, skin, joints, lungs, blood cell formation (leading to low hemoglobin, bleeding problem, susceptibility to recurrent infections).

In end stage lupus nephritis Renal transplantation is very well acceptable treatment with low chances of recurrence in grafted kidney.

But for doing renal transplantation many issues come in to picture like-
1) Availability of compatible doner.
2) Surgery is a major surgery which has its own complication and chances of death.
3) Debilitated general condition of patient due to lupus multiple organ involvement.

So it is better to consult nephrologist and rheumatologist and make decision of renal transplant.
I find this answer helpful
