Hello and welcome to HCM,

Granuloma is a term used for any chronic inflammatory lesion caused by delayed type of hypersensitivity.
Chronic inflammatory reaction can be due to a variety of causes like foreign body reaction, tuberculosis, fungal infection, etc.

Since, there is history of laparoscopic surgery in past, the granuloma could be surgical process or material associated with surgery e.g. sutures.
However, sutures could be a cause if surgery was done in this field.

Thanks and take care
