Before I can give a definite answer I need to know more about your swallowing difficulty.Certain esophageal conditions present with 
chest pain.
How ever, chest pain radiating to jaw ,brings up the concern of cardiac cause.
I also would like to know more about your risk factors for coronary 
artery disease like age, gender, weight, 
hypertension, diabetics mellitus , hypercholestrolemia, family history of cardiac disease, history of medications used, history of 
peripheral vascular disease etc.
I suggest that you need to go the emergency room since the pain lasted for almost 30 minutes and get an EKG done as an initial evaluation.Meanwhile take two 75 mg 
aspirin tablets ,if available at home.
