Is Painful Arms With Pressure And While Twisting Suggest Sprained Bone?

hi, I have a question about the pisiform bone. I have had problems off and on for the last year with this bone. I will twist it wrong and it swells and there is fluid around it. I am not able to touch it or put any pressure on it. If I twist my arm it sends pains up my arm. Is this something I should have checked out or did I just sprain it?
