Is Respiratory Infection Causing Pain In My Ear?

My fiance is complaining of his left ear hurting and he feels like he s going to throw up and says he s burning up. I checked him for a fever but he he s really cool to touch has hardly any warmth to him. Could he have another respitory infection or something else?
