Is Surgery Needed For The Following CT Scan Report?

my brother s daughter have fits on 04.10.2015 and heavy headache. She is 15 years old. She is at Madurai, Tamilnadu. In the early age, she was affected by fits one time. I think this is the second time. On the day, she was immediately admitted in Velammal Medical College Hospital & Research Institute, Madurai. She took rest on the whole week in the hospital. At that time, she took CT Scan, MRI and Angiogram. In CT Scan, the report is ACUTE INTRAPARENCHYMAL HAEMATOMA IN RIGHT TEMPORAL LOBE WITH EVIDENCE OF SURROUNDING EDEMA. NO SIGNIFICANT CONTRAST ENHANCEMENT SEEN. MILD MASS EFFECT WITH NARROWING OF IPSILATERAL VENTRICLE AND MIDLINE SHIFT OF ~2.9mm TO LEFT SIDE. THEY GAVE MEDICATION AS TAB GABAPENTIN 300 MG EPTOIN 100 MG DOMSTAL RANTAC The above medicine will be taken for six months as advised by Doctor and they told it will curable by taking medicine regularly. After taking the above medicines for the past 15 days, she is not eating enough food, got fever, headache, swelling in neck, foot. Kindly advise me any surgery or medicine will cure.

Hi,

Basically she had bleeding inside brain, causing severe headache and seizure. Most commonly this is caused by aneurysm, detected by angiogram (report not mentioned here). Since she improved now, no need of any emergency surgery to remove blood clot. The medicine given is for controlling seizure and not for curing underlying condition. The need and type of any curative treatment later depends on angiogram report. Aneurysms are usually clipped under angiogram guidance to prevent further bleed.

The present symptoms of fever, headache and neck swelling(i guess it is lymph node), in my opinion, is not related to previous event or drug, although we cant ascertain without seeing. It may be viral fever or pharyngitis. If the headache is not severe, no repeated vomiting, and she is active, just try paracetamol for three times for 2 days, and get back to doctor if not getting well.
