The diabetes patient are more prone for development of infection. So you might have lung infection that can cause difficult breathing. The ketoacidosis induced by high sugar also can be responsible for difficult breathing.
You can investigate primary with chest X-ray and CBC. Kindly let me know your sugar value as well.
