my brother has developed disseminated MTB. When he started anti TB drugs (RHZE), he developed hepatotoxicicity within a week time. The ATT was discontinued and 10 days later, SGOT and SGPT returned to normal. But, Bilirubin direct and total are slightly higher (1.9 and 4.6, when the ATT was stopped, BD was 10 and BT 11.5). What do you advise me?

Hi, From the details available, I understand that you already know there was drug related hepatotoxicity. During hepatic cell injury, the enzymes gets released from the cell and therefore the enzyme level gets elevated. Once the drugs were discontinued, the cell injury stopped and the enzyme levels returned to normal within ten days. Return of enzyme levels, indicate that there is no injury going on to liver cells.
However, for the liver to function normally, it requires more time as new cells need to form (cells have died during toxic phase) and manage the metabolic work load. The bilirubin - direct and total are slowly reducing. It should come to normal limits within a month, when the liver starts to function again to its full capacity.
