Is Valsalva The Best For Migraines?

I have a history of migraines and pseudotumor cerebri, low tension headaches as well as run of the mill tension and barometric induced headaches. The migraines and PTC have not been a problem for a long time. I take tizanidine at night and still abuse caffiene. A couple nights ago out of no-where I suddenly felt like meat cleaver was splitting the center of my brain. I found instant relief by doing valsalva, finally found an imitrex and the headache resolved in about 15 minutes. I am very familiar with different headaches and my concern is why the valsalva worked as well as the imitrex, the two seem to contradict each other. The next day the moment I felt a headache starting I took an imitrex and hydrocodone which aborted the headache, a BMP drawn at the office was normal. I do not appear to have any neurological deficits but I am concerned. I have had numerous MRIs (though none in the last decade), and notice no vision changes. I would be very grateful for any insight. Sincerely, Nancy

Neurologist 's  Response
Hi,

First of all, valsalva decreases the volume of blood in the systole,so the blood vessels walls do not stretch. This gives you a sense of relief.Because during the attack of migraine the blood vessels become narrow for a while then the diameter of them increase rapidly resulting in severe pain.

Hope I have answered your query. Let me know if I can assist you further.

Take care.

Regards,
Dr. Mustafa, Neurologist
