Life Expectancy After Lung Transplantation

my grandpa was smoking before but from last 7 years he is not smoking but now he got big problem he is 76 years old and now he has to keep going to the hospital because he cant breath a lot and he cant walk and he got to much weakness even sometimes he cant even walk 10 steps and when he sleeps in his lungs his heart pumps really hard even the bed shakes.doctor says 95% of his lungs are finished. can he change his lungs an live long or what to do ?
