My 16 Month Old Has A Really Severe Red Nappy

My 16 month old has a really severe red nappy rash, it's in the folds of her skin, she cries when I change her nappy even using just water. I was using drapolene nappy cream but it's not getting better. I think it could be fungal as the skin is shiny. Is it safe to use canestan thrush cream (clotrimazole) on this and at her age?
