Initially, it is good that she completed her course of
chemotherapy treatment. In the stage of tumor spread to the brain, we have more than one option. The first option for us is the surgical option: we can operate to remove the
brain metastasis, of course, if the tumor is a single nodule, meaning that on the CT it appeared as a single lesion.
In that case, we can surgically remove it. But if surgery is impossible and there are multiple metastases, we can then do
brain radiotherapy. We give whole-brain radiotherapy. If the metastases in the brain are between one and three nodules, we can go for stereotactic radiotherapy. And it gives same result as surgery.
For treatment afterward, if surgery or radiotherapy is not possible, we can then proceed with
immunotherapy. Immunotherapy, of course, is better than chemotherapy. It has much fewer side effects and much less of the
numbness and neurological signs that were appearing.
The available immunotherapy options are pembrolizumab or Atezolizumab. We can manage treatment with them. They give very good results and have a strong effect in improving the patient’s quality of life. The second thing is that we must, of course, pay attention to her nutrition and hydration. She needs to eat vegetables and fruits . She should have small, frequent meals.
We need to control her pain, which we manage according to her
pain scale. For any neurological symptoms or signs she has, we can give
gabapentin, which significantly improves the pain. Of course, she must also receive a lot of emotional support.
