Hi
The testis is suspended by a spermatic cord which has connection to the abdomen.
So,pain in the testis can radiate to the abdomen.
The lymphatics from the testis drain into lymphnodes in the upper abdomen. So,infection in the testis can have pain in the upper part of abdomen.
Get an ultrasound of abdomen and scrotum and proceed accordingly.
Wish you good health
Regards
