General Surgeon 's  Response
Hi ! Have you had any history of trauma to your lip. In that case, possibly it not a cold sore, but a hemangioma ( growth arising from a vessel), which may be pulsating along with the vascular pulsation.I would suggest to get yourself reviewed by a general surgeon and not to fiddle with it to avoid any bleeding from the lesion. Wishing you an early recovery.
