26 year old male with recurrent attacks of colicky abdominal pain in the epigastric, left lumbar, left iliac fossa pain, associated with fever, diarrhea and vomitting, not radiating, and with no relieving or aggrevating factorsor other GI or GU symptoms,what's the differantial diagnosis?

                                                                this patient was admitted to hospital as a UTI pt, but I thought it was a somewhat strange diagnosis because my pt is a male with no predisposing factors.Anyhow, could it be a case of irretable bowel syndrom with secondary infection?
