Red Dots On Penile Tip Spreading To Foreskin, Painful Urination, Testicular Pain, Itchiness. What Could Be Wrong?

Hey, I have had red dots on the tip of my penis on and off for at least a couple of months, at first I thought nothing of it as they faded quickly, but now they are spreading along the foreskin, at first it was only visible underneath the foreskin around the tip, however it is now visible on the outside of the foreskin. Also, I have began to experience pain while urinating or ejaculating, pain in my testes, and what feels like itchiness on said red spots. I have previously had treatment for chlamydia in the past, but that was over a year ago. I have been in a committed, sexual, relationship for the past 6 months with someone who is clear of any STD's. Please can you give me your opinion on what is wrong, and any advice on what I should do? Thank you in advance.
