Severe Migraines When It Gets Hot. Tried Lemon Water, Digestive Enzymes. Have Low Sodium. Suggest?

At the onset of each summer when it gets really hot I get severe migraines, irrespective of how much water I consume. I take digestive enzymes to help absorb water? to no avail, and squeeze lime/lemon into water, it still does nothing. I also take electrolyte drinks. I have ended up in hospital with it before & they said I had low sodium & had to have checks which I did and all was ok. I take 100mg minax daily and have done for 20 years. I am 61.

Let me tell you that a given medical symptom can be due to various causes and this applies to headache too. Headache can be due to migraine, cluster headache, brain tumor, infection, problem in the blood vessels of the brain and so on. Further the presence and absence of caerian symptoms and the age of the patient is imporatnt in making a diagnosis of patient. I understand that you have long duration of headache and it has a specific precipitating factor. Headache can be multifactorial in your age too. I mean a person can have headache due to many causes. There is a possibility of migraine in you because of long duration of headache. Note that migraine is a diagnosis of exclusion. What i mean, other potential causes should be excluded before this condition is diagnosed. I will approach you as follws
1. you need a bsic neurological assesment which will guide us regarding the need of a scan. If the scan and asessment is normal please follow what i have mentioned below.
2. In view of very long duration of these symptoms the chance of having a serious neurological illness will be low. If your headache is diagnosed as migraine, you will benefit form
a. during acute attack_ simple analgesics. Avoid precipitating factor and avoid stress. Avoid coffee too. As you have told that you are having these problems in certain season and with low sodium, yopu have to check if you are on some medicine that can lower the sodium. Or else you can take juice, lemon water too.
b. Medicines that can make a difference in modifying migraine are available and i believe that this medicines should be used. The options are propranolol, atenolol, valproate, topiramate, amitriptiline etc. Thse medicines should be used for 6 months
